CMHRP: Care Management for High-Risk Pregnancies

The CMHRP program supports pregnant individuals with Medicaid who are at risk for complications such as preterm birth or low birthweight. Our goal is to promote healthier pregnancies and healthier babies by addressing the full range of needs that can affect birth outcomes.
Using a holistic, data-driven approach, our care managers work one-on-one with expectant mothers to:
- Coordinate medical care and prenatal visits
- Address social and behavioral health needs
- Connect families to community resources and support services
- Provide education on pregnancy, childbirth, and newborn care
CMHRP ensures that both mother and baby receive comprehensive, compassionate care every step of the way.
