• • Participate primarily in screening, referring, information gathering, and client follow up.
• Provides injections and immunizations, screens clients, obtains pertinent data, collects vital signs and specimens, assists the physician as needed and prepares and cleans laboratory.
• This position will perform lab duties for Maternity and Family Planning Clinics and also helps with the interview process of clients

Minimum Education And Experience:

Graduation from a State accredited program of Practical Nurse education; or an equivalent combination of education and experience.

Necessary Special Qualifications:

General knowledge of practical nursing techniques in the routine care and treatment of stable, subacute, convalescent, and chronically ill clients; general knowledge of sanitation, personal hygiene, basic health and safety practices applicable to work area. Ability to make accurate observations of clients and families, and to record and communicate this clearly; ability to understand the special physical and emotional problems of clients, and to establish and maintain effective working relationship with them and their families; ability to work with others in the performance of these duties and assisting with other routine tasks of a reasonably high level of complexity.

Description of Work:

• Care Management for At-risk Children (CMARC) is a population management model: CMARC CM’s refer families to community-based interventions for children to maximize health outcomes for children birth – 5 years of age. The program targets the highest risk and highest cost children for care management.
• The responsibilities of this position are: 1) assess children birth – 5 years of age who are identified as having priority risk factors; 2) be assigned to specific medical homes that serve children birth – 5 years of age living in Rutherford County; 3) provide CMARC services based on patient need and according to risk stratification guidelines.
• Identify children at greatest risk through data reports, utilization of risk screening data, provider referrals, and refer to area resources as needed.
• All CMARC documentation is completed on-line in Virtual Health (VH).
• Close collaboration as indicated through phone calls, written updates, and/or office visits with PCP medical homes serving children residing in Foothills Health District.
• Care Managers are responsible to educate families, medical homes and community organizations of the benefits of the program
• Attend staff meetings/In-Services/Webinars/Phone Conferences/Trainings/Community Partner Meetings.

Minimum Education And Experience:

Bachelor’s degree in social work from an appropriately accredited institution; bachelor’s degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor’s degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.

Necessary Special Qualifications:

Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment is required In certain programs or settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial. behavioral, and psychological aspects of client’s problem. Ability to establish and maintain effective working relationships with administrative superiors members of case load and their families, and with civics medical, social and religious organizations. Ability to train or instruct lower-level social workers, students, or interns. Ability to express Ideas clearly and concisely and to plan and execute work.

Description of Work:

• Collects data for history and review of systems, collects laboratory specimens, and performs pregnancy testing.
• Functions in prenatal clinic by providing public health nursing services according to program guidelines.
• Collects data for history, interviews for problems, functions as a laboratory technician by collecting lab specimens by venipuncture, checking blood pressure, heights and weights and urine specimens. Instructs patient and educates to plan of care.
• Performs the duties of an Intake Nurse, (walk-ins, TB tests, head lice check, pregnancy tests, etc.)
• Administer immunizations according to CDC guidelines and ACIP recommendations.
• Answers all incoming telephone calls regarding all program areas when PHN is needed.
• Will assist in other clinics as needed.
• Other job duties as assigned.

Minimum Education And Experience:

Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation, or Master’s in Public Health and graduation from a school of professional nursing; or graduation from a school of professional nursing and one year of professional nursing experience; or an equivalent combination of training and experience. Must have a valid license issued by the N.C. Board of Nursing to practice as an RN in NC, CPR certification and a valid NC drivers license.

Description of Work:

• Nurse Family Partnership (NFP) is an evidence-based, nurse home visiting program that improves the health, well-being and self-sufficiency of low-income, first-time parents and their children.
• Responsible for providing comprehensive community health nursing services to women and their families eligible for the NFP Program.
• Nurse Home Visitors (PHN II) enroll and coordinate services to a caseload of 25 first time, low-income, expectant mothers. Work requires specialized knowledge in the NFP mission, vision and visit guidelines.
• NFP Nurses are required to travel 75% of the time to make home visits.
• Considerable knowledge of and skill in the application of nursing theory, practices, principles and techniques in the application of providing comprehensive community health nursing services to women and their families.
• Considerable knowledge of available resources and organizations and the ability to coordinate these as needed.
• Ability to provide intensive preventive health and social services to women during their first pregnancy and to children birth to 2 years of age.

Minimum Education And Experience:
