CMARC: Care Management for At-Risk Children

The CMARC program is a free care management service designed to support the health, growth, and development of children from birth to age five who are at increased risk for poor health outcomes.

Through personalized support from a dedicated care manager, CMARC helps families:

  • Navigate complex medical, behavioral, or developmental needs
  • Access early intervention services, specialists, and community resources
  • Coordinate appointments and follow-up care
  • Connect with services that support family well-being, such as housing, transportation, and parenting support

Our goal is to give every child the strongest possible start in life by working in partnership with families, healthcare providers, and community agencies.

Eligibility includes:

  • Children with special healthcare needs
  • Children exposed to adverse childhood experiences (ACEs)
  • Families facing social or economic challenges
